Most international development and aid programs involve deliveries of some kind. Relief operations deliver food, medical supplies, and other essential items to those in immediate need. Long term development programs often deliver equipment or supplies to government institutions, or even directly to participants – such as seeds for an agricultural program, or bed nets for malaria prevention.
It is very easy for these types of deliveries to go astray. At various points in the supply chain items can be stolen by those responsible to deliver them. In some cases the items can be delivered, but the recipient can be asked to pay an extra fee even though the items are supposed to be free. In other cases cheaper versions can be swapped for the more expensive ones before the delivery.
To prevent this type of theft it is essential to conduct regular audits of deliveries. This template is a simple form that can be used to record delivery audit results.

This delivery audit template is appropriate when:
- You want to prevent or detect theft on your program.
- You want to check that deliveries of aid or supplies made it to the intended recipients.
- You are auditing a small to medium size program.
This receipt audit template is NOT appropriate when:
- You are auditing a large or complex program.
- The donor has asked you to hire an independent firm to do the audit.
