Supervision Checklist template

Supervision checklists are used during monitoring to verify if an activity has been implemented correctly. They can also be used to give feedback to the person running the activity to help them improve.

For tips and advice, see our guide on how to write a supervision checklist. This template provides a general framework that can be used for any type of supervision checklist.

This supervision checklist template is appropriate when:

  • You need to make a tool to assess whether an activity has been implemented as intended.
  • You need to provide feedback to the individuals implementing activities.

This progress report template is NOT appropriate when:

  • Your organisation or donor already has a standard checklist for the activity (in which case use their template).
